The application instructions and forms are now on-line.  Students applying for a MHSA scholarship will need to download these pdf files, update them using Adobe Acrobat Reader, print them out where they can be mailed or delivered in person.

Instructions to generate and complete the Molalla Alumni Scholarship application On-Line:

The form is in pdf format which you can download to your computer. You will need to have the free Adobe Reader installed on your computer. You can find a copy of the free Acrobat Reader at http://www.adobe.com/products/reader At this time, you may fill in all the information requested.  You can save the partial or  completed form to your computer. At any time, you can print out your copy of the completed form for you to review and submit.  You can save a copy on your computer.

The blank forms pdf file can also be printed off and filled in by typewriter.
